Patient Information

  • Appointments

    Patients are seen by appointment, but walk-ins may be scheduled based on medical urgency and schedule availability. If you must cancel an appointment, please notify us within 24 hours if possible. Patients who arrive more than 15 minutes late to an appointment may have to be rescheduled.


  • Check-in Procedures

    We ask that you bring your insurance information, identification cards and a list of all current medications you are taking. Please arrive 20 minutes prior to your appointment in order to complete the necessary forms. New patients should arrive at least 30 minutes prior to your appointment.


  • After Hours Emergency Care

    For after hours care, call MCH Urgent Care at (432) 640-2749, the Clinics at Walmart West at (432) 332-2695, or East at (432) 362-2685.


  • Lab Results

    Patients will be notified in writing or by telephone of all test results. Please allow 3-5 working days for your results to be returned to the clinic.


  • Payment Policy

    Payment for all services is expected at the time of service. We accept cash, checks, VISA, MasterCard, Discover and American Express at most locations – the Clinics at Walmart do not accept checks.


  • Insurance

    Patients are asked to present their insurance card at each visit. Deductibles, co-pays, etc. are collected at the time of service. For your convenience, most major insurance is accepted and filed. For billing or insurance inquiries call 432-640-1000.

  • Prescription Refills

    We require 48-hours advance notice for medication refills. Your pharmacist may call in your request. An appointment may be necessary before the physician can authorize a refill.
